Success in population health management requires not only a foundation of technology, data and informatics but also relentless effort to better understand a defined population’s outcomes and how they are impacted. Because of this, Providence St. Joseph Health (formerly Providence Health & Services), a five-region health system based in Washington, established an Office of Population Health Informatics within its Population Health division to specifically manage health data coordination, quality measurement, advanced analytics, predictive modeling, and the system’s information technology collaboration.This presentation will describe Providence St. Joseph Health's journey to date with a particular focus on the lessons learned as the health system brought together the people, processes and technology needed to ensure functional and operational performance in support of population health management. Specifically, attendees will learn how Providence St. Joseph Health:
- Determined the complete inventory of Providence St. Joseph Health's population HIT capabilities, including redundancies and gaps; Identified the information technology needed to address gaps and carry out an efficient and effective selection and procurement process;
- Assessed staff performing population health data analytics and related functions to form a common shared service of population health informatics;
- Identified the data requirements and sources, determined consistency and reliability and identified needed remediation to close gaps and improve data quality;
- Defined the desired go-forward technology infrastructure needed; and
- Selected and implemented a care management application to use across the system support managing patients covered by at-risk contracts.
