The Joint Commission Center for Transforming Healthcare, whose sponsors include the American Hospital Association, recently announced targeted solutions to improve communications when hospital patients are transferred between caregivers. Hospitals and health systems that fully implemented the solutions achieved an average 52 percent reduction in communication problems during patient hand-offs. The targeted solutions, which are being pilot tested to prove their effectiveness in demographically diverse hospitals, will be added to the center's Targeted Solutions Tool in 2011.
